Well, as constructive as it gets, it really is a fundamentally well thought out piece that's not just creative, well composed and well-liked piece.
As people who come across my critiques would know, I tend to be more critical and point out flaws, but in this case, forgive me if I only seem to add to the notion of puffery to a concept that I can only say that I wish I had thought out myself.
If there was one thing I could change to make this picture any better and more realistic than it already is, I would suggest changing the dye colour to a brighter shade of red and add small pieces of gel below the strawberry to create the fresh blood in the syringe against the coagulated mess on the table.
This makes me think of that one part of Across the Universe where Jim Sturgess is singing Strawberry Fields Forever by The Beatles. It's really great, definitely caught my eye

"My moral standing is lying down..." -"The Only Time" from the album Pretty Hate Machine by NIN
It's quite intense, the way everything comes together, what with the needle, and the blood, and the very pained fruit…
And that's real blood (read it in earlier comment)? O_O Wow, that's really…intense…
Although, I'm really weird.
I looked at the picture, my focus snapped to the strawberry and the blood on it, and for some weird reason…I though that you put soy sauce on the strawberry. -_-'
Well, lovely shot; this definitely deserved the Daily Deviation!~
